Extended Contract Documentation

Complete the following documentation for ALL contracts.  Disciplinary detention and Credit Recovery contracts will not receive planning time.  All other contracts will receive 5 minutes per hour planning time which must be documented.

• Attendance Sheets (print pages as needed)

• End of Contract Analysis Report

• Parent Survey

• Quarterly Report (required October 30, January 30, and April 30 if contract work during school year)

• Time Sheet

Extended Contract checks will only be issued once all contracted work is completed and all required records have been submitted. 

Payment Schedule

The following are documentation deadlines in order to receive pay for completed contracts:

• ELP documentation due by December 1, 2008, to receive check for completed contract in January

• ELP documentation due by May 6, 2009, to receive check for completed contract on May 28

• ELP documentation due June 30, 200, for any Summer School contracts

This does NOT mean you will be paid on these dates.  Once documentation has been received a request for payment will be forwarded to the Hickman County Finance Office.  For teachers who complete their contracts by December and May, the check will be delivered to you once it has been received in the Office of Teaching and Learning.  Summer school checks may be picked up at the Office of Teaching and Learning one week after all documentation has been received.
